Inspection Request

Instructions:
Please fill in the following information. If you are not sure of the information asked for in a required field, put in your best estimate of the information requested. Please note the following:

Inspections can be requested up to 6:00 A.M. on the day of the inspection. At that time a staff member assigns the inspections to individual inspectors. As your inspection is assigned, a confirmation message will be sent indicating that your inspection has been received and scheduled.

Due to the high volume of inspections and the uncertain length of each inspection, as well as the possibility of staff illness, we cannot make appointments for inspections. However, you can call the inspector on the morning of the inspection between 8:00 and 9:00 A.M.. The inspector will give an estimated window of time for the inspection.
